Join NYC REACH for a free NYS PCMH Sustainability Workshop to learn best practices for maintaining PCMH operations\, how to align PCMH with other programs\, and how to maintain PCMH Recognition. It is important for practices to maintain PCMH operations year-round\, as NYS PCMH Recognition must be renewed annually. \n\n\n\nThis 90 minute workshop is for staff experienced with PCMH.  \n\n\n\nAll workshops will be held virtually in Zoom. The log-in information and how to access the event will be sent in the confirmation that will go to the email address used to register for the workshop.
