Join NYC REACH for the first learning collaborative of 2018! This event offers a unique opportunity for providers to learn about programmatic changes in New York State’s healthcare landscape and resources available to support providers in meeting new goals. Presentations will discuss new opportunities and best practices to be successful in various quality improvement and practice transformation programs. Attendees will also be able to network with NYC REACH content experts.

NYC REACH will also celebrate member practices’ achievements in the Million Hearts initiative, which aims to control blood pressure rates.
