NYC REACH offers free trainings for member practices that have New York State PCMH 2017 Recognition and would like a refresher on the annual reporting process.

NYS PCMH is a New York State-specific PCMH model, and is now the only model available to New York State practices seeking to maintain PCMH Recognition.

• New 2019 updates to NYS PCMH annual reporting standards and guidelines
• Resources available to practices working toward NYS PCMH Recognition
