Provider Information

• ePACES username and password (used to log in to the system)
• CMS Registration ID (note: make sure CMS Registration is up to date)
• NPI on CMS Registration site
• TIN on CMS Registration site
• Payee NPI
• Payee TIN/SSN
• EHR certification number

Program Information

• EHR reporting period start and end date (90-day period)
• eCQM reporting period start and end date (full year)
• Security risk assessment completion date
• Copy of Meaningful Use dashboard report
• Copy of clinical quality measure (CQM) report
• Public health agency names
• Medicaid patient volume (MPV) for the selected 90-day period (numerator = total Medicaid encounters; denominator = total encounters)
