This virtual training introduces medical practice team members to the fundamentals of quality improvement (QI) and team-based care (TBC), with a focus on practical application in chronic care settings. The session explores why quality improvement matters and how structured, team-driven approaches can support meaningful and sustainable improvements in care delivery.
Participants will learn foundational QI frameworks and tools—including the Chronic Care Model, the Model for Improvement, SMART goals, root cause analysis, and plan-do-study-act (PDSA) cycles—and how applying team-based care principles within chronic care workflows can improve care coordination, quality of care, and overall practice performance
