To participate or refer someone, review the workshops in the table below and complete the interest form at the bottom. Please note that the types of workshops available are specified in the first column using the following acronyms:
- NDPP – National Diabetes Prevention Program, a yearlong program with CDC-certified coaches that combines 6 months of 1-hour weekly sessions followed by 6 months of monthly or biweekly 1-hour sessions. Program is designed to reduce the risk of type 2 diabetes.
More info: National Diabetes Prevention Program | CDC. - DSMP – Diabetes Self-Management Program, a 6-week program with certified Peer Leaders that includes 6 weekly 2.5-hour sessions focusing on learning to manage diabetes and reduce related complications.
More info: SMRC – Self-Management Resource Center - DSMES – Diabetes Self-Management Education and Support, offering individual and/or group sessions led by a Certified Diabetes Care and Education Specialist. The program give you knowledge, skills and support to manage your diabetes and lower your risk for developing diabetes-related complications. Learn more.
- NYCCC – NYC Care Calls, a support program with 6 telephonic sessions (30-60 mins each) with trained educators for diabetes sell-management.
- CDSMP – Chronic Diabetes Self-Management Program, a 6-week program with certified Peer Leaders that includes 6 weekly 2.5-hour sessions focusing on learning to manage chronic conditions.
