These materials and handouts will assist with tracking your fitness, food, and weight. Please maintain these materials and handouts up to date. It will help you stay on track to your health goals!
The fitness log will allow you to track your minutes of activity each day and achieve 150 minutes of moderate physical activity per week. Please share this log with your Lifestyle Coach at the start of each session.
