The New York City Health Department encourages adult vaccination providers to use the new Adult Immunization Action Kit to boost vaccinations at their sites. Using the Standards for Adult Immunizations as a framework, this toolkit reviews best practices and includes resources on evidence-based strategies to:

• Assess immunization status of all patients at every encounter,
• Recommend vaccines that patients need,
• Administer needed vaccines or refer to a vaccinating provider, and
• Document vaccines received by patients.

The toolkit also includes health education materials for patients, including a self-screening form and FAQs on adult vaccines, in a number of languages.
