New York State Advanced Primary Care (NYS APC) and the National Committee on Quality Assurance Patient-Centered Medical Home (NCQA PCMH) have merged to create the New York State PCMH 2017 program (NYS PCMH 2017), effective April 1, 2018.
NYS PCMH 2017 is designed to improve primary care and prepare practices for value-based care using the patient-centric medical home model. This model emphasizes care coordination and communication and includes education on value-based care. It also promotes the Quadruple Aim: improve patient experience, improve population health, reduce costs, and improve care team well-being.
