Program Update: New York State Patient-Centered Medical Home 2017

            April 13, 2018

New York State Advanced Primary Care (NYS APC) and the National Committee on Quality Assurance Patient-Centered Medical Home (NCQA PCMH) have merged to create the New York State PCMH 2017 program (NYS PCMH 2017), effective April 1, 2018.

NYS PCMH 2017 is designed to improve primary care and prepare practices for value-based care using the patient-centric medical home model. This model emphasizes care coordination and communication and includes education on value-based care. It also promotes the Quadruple Aim: improve patient experience, improve population health, reduce costs, and improve care team well-being.

Further details about NYS PCMH 2017:

            • Practices that are already PCMH-recognized should participate in NYS PCMH 2017 to sustain PCMH activities.
            • Practices that are currently participating in NYS APC will continue their current activities under the NYS PCMH 2017 program.
            • Practices recognized under NYS PCMH 2017 will receive a per-member per-month reimbursement from New York State Medicaid.

A webinar that provides an overview of NYS PCMH 2017 is available on the NYC REACH resource library in the Practice Transformation folder.

NYC REACH will continue to provide free remote and on-site support. To learn more about this program change and available support, contact pcmh@health.nyc.gov.
