If you were unable to meet the September 30 attestation deadline due to circumstances beyond your control, you may apply for an extension.
To apply for an extension:
- Click here, select the NY Medicaid EHR Incentive Program tab and General Program Information tab
- Download the ADE form (Excel)
- Email the completed Excel form to attestation@health.ny.gov by October 16, 2019. The form cannot be submitted online.
