The New York City Health Department invites you to complete an online survey to be considered for inclusion in the NYC HealthMap’s opt-in directory of LGBTQ-knowledgeable providers.

This online directory is designed to help lesbian, gay, bisexual, queer+ (LGBQ+) and transgender, gender non-conforming and non-binary (TGNCNB) New Yorkers identify affirming health care services. The NYC HealthMap is available online or by calling 311, and includes facility contact information, services provided and populations served.

To be listed as an LGBTQ-knowledgeable provider on the NYC HealthMap, please complete this survey. Providers that complete the survey by Friday, November 1st will be considered for inclusion in the LGBTQ health services directory. Please note that there is a vetting process and completing the survey does not guarantee that your practice will be listed.
