The Extended Access to Care program worked to contribute to the collective impact of Public Health Corps. Community Health Workers provided the following services at clinical sites and community events:

• COVID-19 and flu vaccinations
• Navigation to primary care services
• Primary care services (including screenings for high BP, diabetes risk, depression and loneliness)
• Community Health Worker-led Social Determinants of Health screening and referral to community resources
• Off-site community vaccination events
