About This Event
This training covers evidence-based strategies to prevent, identify, and manage hypertension and cholesterol, using clinical workflows, EHR tools, quality measures, and patient engagement to improve cardiovascular outcomes such as; hypertension for Best Practices to diagnosis, treatment, and monitoring, including lifestyle counseling, medication management, blood pressure measurement techniques, adherence strategies, patient self-management, and identifying undiagnosed hypertension through EHR reports and follow-up workflows. Additionally, tips for cholesterol screening, risk assessment, statin eligibility, lipid management, and EHR configuration to support alerts, labs tracking, and reporting. Also, will look into quality & Population Health to learn about cardiovascular quality measures, documentation requirements, EHR optimization, care gap identification, risk stratification, and proactive panel management to address overdue visits, missing labs, and medication gaps.
