About This Event
This training will review the importance of diabetes prevention and management, including identifying and addressing prediabetes, diabetes screening, and evidence-based diabetes management strategies. Participants will learn how to organize diabetes ICD-10 diagnosis groupings, manage patients with poor diabetes control (e.g., A1c> 9%), and use population health and panel management approaches to identify care gaps and support high-risk patients. The training will also cover chronic kidney disease (CKD) screening in patients with diabetes, including the importance of annual screening, National Kidney Foundation guidelines, CKD laboratory ordering and tracking, patient education, patient lists, and referral workflows. In addition, participants will review diabetic retinopathy screening guidelines, the importance of annual eye exams, how to create retinopathy screening procedures, referral tracking and follow-up processes, and patient education strategies to support the timely completion of screenings.
