CHIP Learning Collaborative  

This meeting will be an online event and will include a presentation and discussion with NYCREACH staff who will be discussing an introduction and overview to the CHIP Learning Collaborative, HRSN screenings, Social Care Network (SCN), and history and future developments for the CHIP GIS tools. The agenda and any pre-readings will be sent closer to the meeting date. 

We look forward to an exciting kick off to CHIP Year 3 and to joining our new and returning members in valuable discussion!
