About This Event
This interactive virtual training is designed to help medical practice staff make more effective use of MDLand documentation and care planning tools. Participants will gain practical insight into building efficient chronic care workflows and how they can contribute to improving patient chronic care management through workflow redesign and quality improvement activities in their practice. The session will also highlight Health Registry Reports to identify care gaps and monitor patient outcomes, managing electronic referrals and tracking follow-up care, documenting chronic care visits, assessments, and follow-up plans, the use of templates to improve documentation and workflow efficiency, documenting preventive screenings and follow-up needs, clinical reconciliation to reviewing medications, allergies, and problem lists, use CDS tools and order sets to support guideline-based care, providing and documenting patient-specific education, reviewing coding and procedure documentation for chronic care, and managing hypertension and diabetes through monitoring, counseling, and follow-up.
