NYC DOHMH DSMES Stakeholder Convening Summer 2026

Please join the NYC Health Department for an opportunity to connect with fellow diabetes educators from across the city. This convening will feature general updates and a special presentation by the NYC BOLD Program:

The NYC Building Our Largest Dementia (BOLD) infrastructure Program is a CDC-funded initiative that addresses brain health for all New Yorkers. The program seeks to educate the public about dementia risk reduction, increase the health status and quality of life of NYC residents affected by dementia, and improve the lives of their caregivers. The program sits within the Center for Health Equity and Community Wellness within the NYC Health Department. It is comprised of programmatic staff and a coalition, made up of internal and external partners, and is informed by the CDC’s Healthy Brain Initiative State and Local Public Health Partnerships to Address Dementia: 2023 – 2027 Road Map. Our mission is to foster a collaborative ecosystem that provides compassionate care and supports for individuals at risk for and diagnosed with dementia, their loved ones, and their caregivers. Our vision is for all New Yorkers at every life stage to have access to information and services for primary, secondary, and tertiary prevention of dementia.
