Healthy Hearts Teams uses practice facilitation, also referred to as quality improvement coaching, to help smaller independent practices adopt team-based care (TBC), a proven strategy to improve hypertension (HTN) control. This initiative aims to:
- Apply core components of the team-based care model to HTN workflows.
- Maximize the expertise of every member of the practice care team to deliver quality hypertension care.
- Improve blood pressure control rates in patients with HTN.
- Increase sustainability of practice TBC HTN workflows.
This project is a partnership with the NYU School of Global Public Health. The enrollment period for this project has ended.
