The NYC Health Department set the HealthyNYC goal of reducing maternal mortality by 10% by 2030. Reaching this goal will take a team effort across the NYC healthcare landscape. NYC REACH offers maternal health programming for primary care providers, health centers, and doulas.

- No-cost technical assistance, workflow support, and tailored staff training
- Unrestricted milestones payments up to $25-50K for each health center
- Help to meet new UDS reporting requirements related to screening for family planning needs
PRECONCEPTION CARE IN THE PATIENT-CENTERED MEDICAL HOME: If your organization is recognized as a Patient-Centered Medical Home (PCMH), we invite you to attend our Preconception Care in PCMH training. The next training date is March 5, 12-1:15 PM. Preconception care means understanding your patients’ reproductive plans, and for those patients who are interested in becoming pregnant, working with them to prepare for a healthy pregnancy. By prioritizing care before conception, we aim to improve health outcomes and promote overall well-being for individuals and families across the city. Learn more and register here.
