Doc AIM is a free integrated pest management (IPM) service for children and adolescents (0-21 years old) diagnosed with asthma. IPM is a non-airway irritant method of pest management and has been proven to reduce asthma symptoms and hospitalizations. Doc AIM partners with CABS Health Network to provide timely coordination of IPM services.
To be eligible for this program, the child or adolescent must have the following:
- Reside in NYC
- Stable housing
- Meet 1 of the 2 clinical criteria:
- A recent hospitalization or observation stay with a diagnosis of asthma
- OR at least 2 hospitalizations, urgent care, or emergency visits within the last 12 months, with a diagnosis of asthma.
- Stable housing
Immigration, insurance and landlord consent are not required.
There are multiple ways to refer your patients to the Doc AIM program:
- By Email or Fax: Fill out a PDF referral form. This can be faxed (718-889-7113) or emailed (cabsMTIA@cabshomecare.org) to CABS Health Network with the title “Doc AIM Referral.”
