            Doctors for Asthma Integrated Pest Management (Doc AIM)

            In partnership with CABS Health Network, Doctors for Asthma Integrated Pest Management (Doc AIM) provides integrated pest management services to children (0 to 17 years old) diagnosed with asthma. To be eligible for this program, the child must have the following:

            • Stable housing
            • Residing in NYC
            • 1 of the 2 clinical options:
              • A recent hospitalization or observation stay with a diagnosis of asthma
              • OR at least 2 hospitalizations or emergency visits within the last 12 months, with a diagnosis of asthma.
            • No insurance required
            • No landlord consent required

            There are multiple ways to refer to the Doc AIM program. After filling out an excel or PDF form, this can be faxed (718-889-7113) or emailed (cabsMTIA@cabshomecare.org) to CABS Health Network with the title “Doc AIM Referral.”

            If you use the Unite Us Referral Platform, you can refer to CABS Health Network by searching “Doc AIM” and filling out the form through a referral.
