            The National Diabetes Prevention Program (National DPP)

            The National DPP is a yearlong lifestyle change program, recognized by the Centers for Disease Control and Prevention (CDC), designed to help participants prevent or delay the onset of Type 2 Diabetes. READ MORE>

            The National DPP is a yearlong lifestyle change program, recognized by the Centers for Disease Control and Prevention (CDC), designed to help participants prevent or delay the onset of Type 2 Diabetes.

            The program includes 16 weekly 1-hour core sessions, followed by 10 bi-weekly/monthly 1-hour maintenance sessions. Sessions are facilitated either virtually, in-person or in combination by at least one (1) certified lifestyle coach who follows a CDC approved curriculum.

            NYC REACH partners with clinical and community organizations to implement the National DPP in New York City communities and neighborhoods with high rates of prediabetes. Engaged organizations receive training and technical assistance that involves: 

            • Staff certification as CDC Lifestyle Coaches and ongoing training in motivational interviewing techniques, group facilitation, and nutrition
            • Access to program materials and tools for program implementation and data management.
            • Monthly sessions with a Program Specialist to:
              • Apply for CDC program recognition
              • Develop a strategy for community outreach and referral pathways
              • Tailor workshop sessions to meet participants needs
              • Integrate health related social needs screening and referrals to support lifestyle behavioral changes and increase participants engagement
              • Seek reimbursement through Medicaid and Medicare DPP for National DPP sessions attended by eligible participants.
