This program builds capacity for social service referrals and community and clinical linkages by subsidizing licenses for Unite Us for city-led programs and NYC REACH partners including community-based organizations and clinical practices. NYC REACH provides technical assistance, a quality-improvement-driven curriculum, and trainings to optimize utilization of the platform.
