Take Control aims to improve access to and utilization of DSMES services in New York City, to ultimately help reduce diabetes related health disparities, via direct delivery of DSMES in settings where access is limited (TRIE neighborhoods and NHACs) as well as support for NYC providers seeking to achieve or maintain DSMES accreditation.
