NYC REACH is expanding its services for primary care practices during the COVID-19 emergency. NYC REACH Membership is not required to receive the following:

• Technical assistance with telehealth
• Support with identification and outreach to high risk patients
• Updates on potential funding opportunities and policy changes
• Access to a provider survey to share concerns as they evolve

Primary Care Practice Survey
New York University is conducting a survey to learn about the challenges facing independent practices during the COVID-19 public health emergency. This information will be used by New York City to advocate and seek support for practices. Contact us to learn more.

Multiple entities are offering financial aid and relief to health care providers. Click here for a running list of potential funding opportunities for your practice.

Patient Outreach
NYC REACH practice facilitators can assist your practice with workflows and best practices for identifying and contacting patients vulnerable to COVID-19. If your practice is unable to conduct patient outreach, NYC REACH operates a patient call center that can help. Bilingual staff can inform patients about services available at your practice, provide general information about COVID-19, and assess needs and provide resources for medications, food, housing, health care access, and mental health. Note: Practices must have NYC REACH membership to receive support from the patient call center. Contact us to learn how to become a member.

COVID-19 Hotel Program 
The City is offering free hotel rooms for people who need to self-isolate but are unable to do so in their current living situation.  Hotel rooms will be offered based on referrals from NYC Health + Hospitals and community health care providers. Visit the program page here.
