CONTRACEPTIVE CARE IN PRIMARY CARE FOR FQHCs: If your organization is a Federally-Qualified Health Center or Lookalike, and you are looking to implement or improve contraception care workflows, let us connect you to our expert partners at Upstream. Review the program flyer for more information. Contact your practice facilitator or nycreach@health.nyc.gov for more information. Participants in this program will receive:
- A team to help your health center offer the full range of contraceptive services in primary care
- No-cost technical assistance, workflow support, and tailored staff training
- Unrestricted milestones payments up to $25-50K for each health center
- Help to meet new UDS reporting requirements related to screening for family planning needs
