NYC REACH works with a panel of Community Health Workers (CHWs) to introduce social needs screenings in independent primary care practices and provide referral connection for their patients to community resources providers.
The CHWs have become a trusted member of the practices they serve. We look forward to continuing this work to introduce CHW involvement in other areas of care, including hypertension self-management and behavioral health integration. To learn more about this program, email sseeram@health.nyc.gov.
