New York State (NYS) Medicaid has proposed to change the reimbursement incentive payments for providers working at Patient-Centered Medical Home (PCMH)-recognized practices. A detailed breakdown of incentive payment changes are available here.

Based on the announced proposal, the first set of incentive payment changes will be in effect from May 1 to June 30, 2018. A second set of incentive payment changes will begin July 1, 2018 and will align with the principles of value-based payment (VBP).

Additional guidance and educational materials on the incentive payments will be published in the coming months. NYC REACH will continue to update members on these changes.
