The New York City Department for the Aging (DFTA) is hosting free Diabetes Self-Management Program (DSMP) workshops for patients.
The DSMP is a free, evidence-based course that provides participants with the tools and knowledge to help manage their diabetes. The program is for adults aged 60 or older diagnosed with type 2 diabetes and is offered at select senior centers throughout New York City.
These workshops will begin in October and take place over six weeks. Workshops will be held in English or Spanish. Space is limited and pre-registration is required. Program brochures are available here in English and here in Spanish.
Please contact Robert Fay at RFay@aging.nyc.gov with any questions about DSMP. For information about a specific workshop, contact the corresponding phone number listed below.
