The SMRC programs are evidence-based self-management programs that provides participants with the tools and knowledge to help manage their chronic health conditions. Programs includes the Chronic Disease Self-Management Program (CDSMP) and the Diabetes Self-Management Program (DSMP).

The programs lasts 6 weeks and involves 2.5 hours weekly sessions. Sessions are facilitated either virtually or in-person by two (2) trained Peer Leaders who follow a SMRC approved curriculum. 

NYC REACH partners with clinical and community organizations to implement SMRC programs in New York City communities and neighborhoods with high rates of hypertension and diabetes. Engaged organizations receives training and technical assistance that involves: 

  • Staff certification as CDSMP and/or DSMP Peer Leaders and ongoing training in motivational interviewing techniques, group facilitation, and nutrition
  • Access to the New York City Department of Health SMRC License at no cost to deliver DSMP and/or CDSMP workshops.
  • Access to program materials and tools for program implementation
  • Monthly sessions with a Program Specialist to:
    • Develop a strategy for community outreach and referral pathways
    • Tailor virtual and/or in person workshop sessions to meet participants needs
    • Integrate health related social needs screening and referrals to support lifestyle behavioral changes and increase participants engagement
