CHIP is a 5-year initiative focusing on comprehensive efforts to identify and respond to health care disparities in cardiovascular disease and improve related outcomes, specifically for those with Hypertension (HTN). Interventions will assess and address the disparities and inequities in communities at highest risk, where there is a particular need for equity-focused health system interventions to prevent, detect, control, and manage HTN.
