The patient engagement call center aims to increase referrals to evidence-based programs designed to support the adoption of a healthy lifestyle for the prevention and self-management of chronic disease conditions such as hypertension and diabetes.

Participating organizations provide the call center a list of eligible individuals they want to connect with prevention and self-management programs. NYC REACH staff calls patients on behalf of the organization to share information about available programs based on eligibility. Interested individuals are enrolled and connected with organizations delivering the program of interest. NYC REACH provides semiannual reports on outreach results to participating organizations.
