With the end of the Public Health Emergency declaration and the commercialization of COVID-19 vaccines, this program continues partnerships with Federally Qualified Health Centers and other clinics to ensure equitable access to vaccines, including COVID-19, flu, and routine childhood/adult vaccinations. Specifically, NYC REACH supports practices to:

-Provide outreach to patients that are regularly seen at the clinic and haven’t received vaccines for COVID-19 and flu.

-Assess patient vaccination status of other recommended vaccines and provide vaccination services on site during the encounter, or by setting a future appointment.

-Assess and address additional healthcare needs for each patient vaccinated & offer applicable resources.
